Summary:
Under the direction of the Outreach and Engagement Supervisor, obtains initial participant information and referrals to community agencies as appropriate.
Promotes the PACE program in the community to potential participants and other agencies.
To include presentations to potential referral sources such as community groups; develops new referral sources and maintains referral tracking system and marketing file.
Qualifications and Requirements:
Education:
High School Diploma or GED required.
Associates or Bachelor's Degree preferred.
Experience:
Minimum of 2 years' experience in a healthcare, insurance or similar environment.
Working with states Medicaid and other financial systems preferred.
Computer proficiency in word and excel applications.
Minimum 1 year working in senior healthcare is preferred.
Marketing or sales experience preferred.
Licensure/Certifications:
Valid New Jersey Driver's License, proof of auto insurance and consistent and reliable usage of a motor vehicle.
Skills and Knowledge:
Must have consistent and documented experience in health care or geriatric marketing.
Knowledge of customer service and marketing principles and practice.
Extensive knowledge of community resources available to seniors and their families.
Ability to effectively represent the program to participants, families, outside agencies, community groups, community professionals and the general public.
Excellent verbal and written communication skills.
Excellent public speaking skills for large and small groups.
Ability to work independently and exercise sound judgment.
Ability to establish effective relationships with community members, referral sources and co-workers.
Ability to maintain accurate records.
Ability to effectively present PACE philosophy and model to the community and prospective participants.
Working knowledge of physical, psychosocial, behavioral and family needs of the elderly.
Working knowledge of community health and social service delivery systems and aging networks.
General knowledge of financing mechanisms such as Medicaid and Medicare Good working knowledge of PACE philosophy and eligibility/denial criteria.
Strong organizational and time management skills with the ability to schedule appointments.
Able to work independently and function with minimal supervision.
Medical Clearance:
Must have medical clearance for communicable diseases and up-to-date immunizations before having direct participant contact.
Duties and
Responsibilities:
Receives and provides information to referrals to the PACE program; refers individuals to other appropriate resources as indicated.
Provides individual and group marketing presentations to potential referral sources following State and Federal guidelines.
Meets or exceeds census enrollment targets.
Maintains consistent, close contact with existing referral sources.
Develops and cultivates new referral sources.
Maintains customer focus and serves as community liaison.
Maintains referral tracking system and mailing list as well as marketing files.
Participates in marketing activities, e.
g.
senior fairs, canvassing senior hi-rises, as developed in marketing plan.
Plans and coordinates open houses/marketing events to enhance exposure for Beacon of LIFE PACE program.
Must communicate referral information received to enrollment specialist and report outcomes to Marketing Supervisor.
Contacts identified key referral sources regularly to discuss possible program candidates Maintains ongoing relationships with identified individuals and agencies to ensure a clear understanding of PACE as a vital community resource.
Shares responsibility with other team members in the implementation of the PACE philosophy and achievement of program goals.
Attends staff meetings and in-service training as required.
Follows all Beacon of LIFE Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
Practices Standard precautions and follows appropriate Infection Control procedures.
Maintains safe working environment.
Participates in and supports the Quality Improvement initiatives.
Participates in continuing education classes and any required staff and training meetings.
Maintains professional affiliations and any required certifications.
Performs other duties as required or requested.
